Requirements and Contacts
1. Register for ASHP match by January 13th, 2012
2. Application Deadline January 31, 2012
- Completion of a PharmD program. Send most recent official school of pharmacy transcript by US Mail.
- Personal statement on why you want to complete a residency at NorthBay Healthcare. Attach your CV and your Personal statement in one document when prompted to add a resume while applying online [see part 3 below].
- At least 3 letters of recommendation. Include at least one recommendation each from a preceptor of a clinical APPE and one from an employer. The ASHP form may be used alone or in addition to a letter of recommendation. Request the letters to be sent by email to pharmacy.residency@northbay.org or you can mail them to the following address:

To apply for the program, you'll need to use our online job application form.
- Register and create your own profile
- Once you have completed the registration process, search for "Pharmacy Resident" in the "Allied Health" Category
